For seniors transitioning out of nursing homes, hospitals, or shelters, our program prioritizes immediate stabilization by addressing their emergency needs first. Whether providing a hot meal, clean clothing, or removing any immediate barriers to placement, we ensure a seamless transition into stable housing. Our dedicated Case Managers offer rapid, on-the-spot support—personally transporting the senior to their chosen residential unit and conducting a thorough housing quality inspection prior to move-in. To guarantee a secure transition, the Case Manager remains present throughout the entire lease-signing and move-in process. Additionally, through established community partnerships, we assist clients in acquiring essential furniture to fully establish their new homes.
